Doctor Strange in the Multiverse of Madness is now running in theaters, the third eye is introduced firstly in Sinister Strange on his forehead, he is a darker version of Doctor Strange who was corrupted by Darkhold. The third eye will increase Strange’s mystical powers and a horror vibe.
At the end of Doctor Strange 2, Strange controlled the evilness of Darkhold and as a result, he got his own Third Eye on his forehead, Speaking to Rolling Stone, Doctor Strange 2’s head writer Michael Waldron told explained the end scene in which Strange gets a third eye in his forehead.
Waldron told “I felt like we had a happy ending. We were like, like, ‘Gee, you know, for a movie where a lot of bad shit happens, we got kind of a happy ending here.’ We really wrapped it up and that didn’t quite seem right. We kept thinking about what Mordo warned Strange in the first movie: ‘The bill comes due.’ It’s like Wong says, ‘You possessed your own corpse. Like, is this guy ever going to face any consequences? And it just felt like a great nod to horror movies where there’s that final twist.”

Doctor Strange’s Third Eye of Horus:
In Marvel Comics, the third eye of Doctor Strange is called ‘Third Eye of Horus’, The Third Eye of Horus was an item given to Beast by Doctor Strange during an emergency to allow the X-Man to see the Shadow Creatures of the Twelfth Dimension. The first use of the Third Eye caused Beast to pass out. When he used it a second time, words spoken by Strange earlier allowed him to grasp a better control of the item and see the magical creatures in an appearance more suited to his scientific mind. After exiling the Shadow Creatures, Strange let Beast keep the mask.

“It was doubly exciting. It’s like, well, what if Strange has embraced this darkness a little bit? He’s figuring out how to live with this. What does that mean for our hero? That’s an interesting question to pose on its own. Now here comes Clea saying, ‘You caused an incursion. You did the thing— perhaps the very thing—you were hoping Wanda wouldn’t do. Now we’ve gotta fix it together.’ So, yeah, it just felt like a nice little tease of adventures to come.”
